Who We Support
Specialist Support for Mental Health, Learning Disabilities, and Autism
Behind Every Referral Is a Person With a Story Worth Telling.
We support adults across a wide spectrum of conditions and life circumstances. What every person we support has in common is this: they deserve more than a placement. They deserve a partner in building the life they want.
Condition Profiles
Thoughtful, individual support, never a one-size-fits-all response.
Mental Health
Mental health conditions affect people in profoundly individual ways. Two people with the same diagnosis may need entirely different kinds of support. We approach mental health with that understanding at the forefront, building support around the person, not the label.
Conditions we regularly support include schizophrenia, bipolar disorder, PTSD, complex trauma, depression, anxiety disorders, EUPD and OCD. We support adults at different stages of their recovery journey, from those taking their first steps toward independent living after a long inpatient admission, to those maintaining stability and building toward a life of genuine independence and purpose.
Referral Pathway
How a Referral Works, From First Contact to First Day
- 01
Step 1, Reach Out
Call us, email us, or complete our referral form online. We aim to acknowledge every enquiry within four business hours and to provide a full initial response within one working day.
- 02
Step 2, Initial Conversation
We will speak with you, the referrer, to understand the individual's background, current situation, needs and goals. We are direct about what our service can and cannot offer, because the right fit matters more to us than filling a vacancy.
- 03
Step 3, Pre-Placement Assessment
We conduct a thorough assessment of the individual's needs, risks and strengths, reviewing documentation, speaking with existing professionals and, wherever possible, meeting the person in their current environment.
- 04
Step 4, Support Plan Co-Production
Before anyone moves in, we co-produce a detailed support plan with the individual, their family and relevant professionals. Placement commences only when we are confident the plan is right and the environment is ready.
- 05
Step 5, Transition and Settling In
We invest deliberately in transitions, supporting the person through every stage of moving in, meeting their support team and beginning to feel genuinely at home. The first weeks of a placement are often the most important, and we treat them accordingly.
- 06
Step 6, Ongoing Review
We review every support plan formally at minimum every three months, or sooner following any significant change or incident. Reviews involve the service user, their family, their support team and relevant commissioners or health professionals.
